Family Health Plus

Available to: single adults, couples without children, and parents; must be aged 19-64, a resident of NY State and a U.S. citizen or fall under one of many immigration categories.

Services included: regular medical check-ups, prescription drugs, hospital care, eye exams, eyeglasses, mental health services, emergency services, diabetic supplies & equipment, dental care, family planning & reproductive health services

Child Health Plus

Available to: children under the age of 19 who are residents of NY state who do not qualify for Medicaid.

Services included: well-child care, physical exams, immunizations, illness treatment, emergency care, surgery, inpatient hospital care, prescription drugs, dental care, vision care.

Healthy NY

Available to: NY State residents currently employed (or within the past 12 months), not provided with insurance through employer, ineligible for Medicare, have not had insurance for past 12 months or have lost insurance due to a recent event (for example, change of employer).

Services included (with varying co-payments): preventive & primary health care, inpatient & outpatient hospital services, emergency services, maternity care, physical therapy, diabetes equipment & supplies.

Services not covered: mental health services, alcohol & substance abuse treatment, chiropractic, hospice, ambulance, dental & vision care.
